I might be wrong but it looks to me as if the nose is wider towards the front and with a more pronouced waist than previously.

It does look like it is skinnier around the camera pods, yeah

It depends and an answer isn't as clear cut:
-A winglet underneath the exhaust will help in an upwards-bending exhaust gasses flow, benefiting the upwash effect and thus interacting between diffuser and rear wing, BUT because the exhaust flow is low pressure and you blowing it on top of the winglet, you are not creating downforce with it, even inducing lift.

-A monkey seay above the exhaust will make it create extra downforce since you are blowing the low pressure exhaust underneath the wing. The exhaust flow will tend to follow the upwards bending pattern of the winglet due coanda effect, but will be less effective then the other solution, thus the upwash effect will be less. And of course a full fletched monkey seat creates more drag then a simple winglet tucked in behind existing bodywork.

It should be a simple matter of where that particular team gets more clean air to. The fact that RedBull are positioning it lower suggests that they're getting more clean air around the bottom of the car than other teams.

They have quite a bit of free area under the exhaust which would pretty much be going to waste otherwise.

It's an interesting difference between other teams. Pretty much everyone has raised this area to provide more area under or around the gearbox, while Red Bull has kept the lower gearbox

Just an aside about blowing the topside of monkey seat. The exhaust stream is high velocity and low pressure but if blown on top of a monkey seat it would stagnate and form fairly high pressure. You can't blow a stream of low pressure only a stream of gas and it's pressure doesn't stay low once it slows.
